Kankakee School District Athletics Eligibility:
Any student interested in playing sports must have a valid physical on file with the main office or athletic office of the school.
Kankakee School District requires all student-athletes have an agreement to participate on file with the main office or athletic office of the school.
Kankakee School District requires all student-athletes either purchase school insurance or sign an insurance waiver. This form must be notarized (a service provided free at the KHS Athletic Office) and kept on file in the main office or athletic office of the school.
School Insurance can be purchased through any of the District schools.
Governor Quinn signed a bill into law in August of 2011 that requires all school athletes be educated about concussions. Each student-athlete must have a sign concussion form on file in the main office or athletic office of the school.
IHSA Drug Testing Consent Form
The IHSA requires all high school athletes sign a consent form regarding mandatory random drug testing. The signed consent forms must be kept on file in the main office or athletic office of the school. The links to these are as follows:
